Comfort Keepers Geriatric Care Management Services in Federal Way, WA
Your senior loved one may benefit from Comfort Keepers professional Geriatric Care Services
At Comfort Keepers In-Home Senior Care of Federal Way, WA we look at geriatric care management as a way of serving seniors holistically—to care for their physical, emotional, and practical needs using our care managers’ expertise and heart to provide the best possible solutions for seniors and their families.
Our home care agency's geriatric care management services go above and beyond just exceptional caregiving; it includes being a liaison between service providers such as doctors or attorneys and seniors and their loved ones, as well as equipping families with resources to better care for the elders in their lives. Our care managers help with the varied issues seniors face such as crises, like hospitalizations, the loss of a spouse, or depression. We understand dealing with the changes that come with aging, such as increased isolation, discontent with the quality of care being received. With all these factors, families become overwhelmed with the amount of care needed; life transitions, such as moving or a hospital discharge; and long-term planning, regarding things such as advance directives, living wills, or long-term insurance.
At Comfort Keepers In-Home Senior Care of Federal Way, WA, we believe wholeheartedly in the importance of advocacy and education as a way to improve the health and wellness of seniors’ lives. We have had many chances to put this into practice while caring for our clients.
One previous client, in particular, Dorothy, is a prime example of this. Dorothy’s POA lived out of the area and, unlike our caregivers who were there on a daily basis, was largely unaware of the issues her aunt was facing as her dementia increased and her body weakened. Due to a large health crisis and continued mental instability, Dorothy was a prime candidate to be moved out of her independent living facility and into something with a higher level of care. However, because she had no family to advocate for her in this area, it was vital that we stepped in. After coordinating with doctors, housing managers, and long-distance with the niece, we were able to help recommend a plan of action for Dorothy that would ensure her long-term health and safety.
We would love to do the same for you! Geriatric care management can be effective in any situation involving a plan for the future of a loved one but is especially recommended if you don’t live locally, are overwhelmed by the flood of decision-making, are not able to personally be at an important doctor’s appointment, or just need some direction in the care of your loved one.
Some of the services we offer include:
- Someone to listen and ask insightful questions at important doctor’s appointments
- Help with living arrangements, communicating with social workers and discharge planners
- Working with physicians, attorneys, and senior living communities
- Coordinating with family members, especially those out of the area
- Above all, we believe in providing resources so that every need of a senior can be met
