How it Works

Step 1 - Comfort Keepers will set up an initial consultation.  This is the first step to developing a customized plan of care.  During the consultation, we will lead you through a conversation pertaining to your loved one’s health, personality, and living environment.  This will help us determine what programs will best meet their needs. 

Step 2 - We will then combine your input and guidance to develop goals to help maintain independence and quality of life.  Based on the consultation, and goals. We will present you with a customized care plan specifically designed to meet your loved one’s needs and lifestyle.  We review this plan together along with your billing details. 

Step 3 - Following the customized plan of care we develop together, your loved one's care team will begin to deliver the care and programs. 

Step 4 - Comfort Keepers understands that life changes.  The programs are outlined to evolve as your loved one's needs change.  Your Client Care Coordinator will conduct ongoing reassessments and evaluate progress towards your goals.  This is to ensure that the customized plan of care is delivering exactly what your loved one needs. Reassessment will be conducted, and care plan goals evaluated upon any significant health, wellness, family, or support system changes.  This will ensure the care plan evolves to reflect the current situation. 

Step 5 - Ongoing Communication will continue throughout the entire process, the Comfort Keepers care team will be in frequent communication with your family.  Family members will also have access to the web-based family communication center to review schedules, care plans, assessments, and caregiver logs.
