Medicaid/Veterans' Care


Does Medicaid cover Home-Care Services? Yes – under the Community Care Program. Comfort Keepers has been supporting Community Care Program since 2003 and has provided home-care assistance to families in need in Cook, Lake and DuPage counties. Please call us at (847) 447-2211 to learn more. 

Community Care Program

Older adults in Illinois ages 60+ who receive Medicaid may be eligible to receive additional services through the Community Care Program (CCP). This program is designed to prevent unnecessary institutionalization of older adults who have difficulty with household and personal care tasks. Core services include in-home, adult day and emergency home response.

The Community Care Program provides services to any person who applies for the program and meets all current eligibility requirements.  The Community Care Program is one of the 1915(c) waivers for home and community-based services under the Medicaid Program.  For more information, visit the Illinois Department on Aging at https://www2.illinois.gov/aging/programs/ccp/Pages/default.aspx


Who is eligible?

People who...

  • Are 60 years old or older; 
  • Are U.S. citizens or legal aliens; 
  • Are residents of Illinois; 
  • Have non-exempt assets of $17,500 or less (Your home, car, or personal furnishings are classified as exempt assets.); and 
  • Have an assessed need for long term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment).


What is the process of applying?

  1. Reach out to a Care Coordination Unit (CCU) to be assessed for the Community Care Program, along with other programs and benefits that may be available to you or your loved one.

Care Coordination Units are appointed by Illinois Department on Aging to provide local support to communities and help reach all that seek assistance.

  1. Once assessed by a Care Coordination Unit (CCU), and deemed eligible for in-home home care, you and your loved one have a choice of the home-care agencies that can provide a caregiver (also known as homemaker).
