What Is the Community Care Program?
The Illinois Community Care Program (CCP) helps older adults remain safely and independently in their homes rather than moving into a nursing facility. The program provides services for eligible individuals who need assistance with daily living activities and household tasks.
Services may include:
- In-home caregiver assistance (homemaker services)
- Adult day services
- Emergency home response services
The program serves individuals who meet current eligibility requirements and is offered through Illinois Medicaid under a Home and Community-Based Services waiver.

Who Is Eligible?
You may qualify for the Community Care Program (CCP) if you:
- Are 60 years of age or older
- Are a U.S. citizen or qualified legal resident
- Live in Illinois
- Have non-exempt assets of $17,500 or less
- Meet the state's Determination of Need (DON) assessment criteria for long-term care services
Note: Your home, vehicle, and personal belongings are generally considered exempt assets.
