Do you Qualify?
- Military Service: At least 90 days active duty, with at least one day during a wartime period (WWII, Korea, Vietnam, Gulf War).
- Discharge Status: Other than dishonorable.
- Care Need: Requires assistance with daily activities (bathing, dressing, eating, mobility) or is housebound or in a care facility.
- Income & Assets: Income and countable assets fall within VA thresholds. Unreimbursed medical and care costs can reduce your "countable income."
How to Apply - Step by Step
- Gather your documents: DD-214 (discharge papers), medical records documenting your care need, financial statements, and marriage or death certificates if applicable.
- Submit VA Form 21-2680: Your physician completes this Examination for Housebound Status form. Comfort Keepers can connect you with a VA-accredited claims agent or VSO at no charge.
- File with the VA: Submit VA Form 21-534EZ (surviving spouse) or 21-527EZ (Veteran) along with supporting documents to your regional VA office.
- Start care while you wait: Processing can take several months. We can begin in-home care services right away — and payments are often retroactive to the filing date.
