VA Community Care Network

Comfort Keepers In-Home Care in Bedford, Texas.

CCN authorization can cover your home care at no out-of-pocket cost

The VA Community Care Network (CCN) helps eligible Veterans receive care from approved community providers when the VA cannot provide the care needed directly or when care in the community better fits the Veteran’s needs and circumstances. Comfort Keepers® of Euless is proud to support local Veterans and families with compassionate in-home care services that help promote safety, independence, and quality of life at home.

What CCN Covers Through Comfort Keepers

Through the VA Community Care Network, eligible Veterans may be able to receive care from VA-approved providers in their local community. Community care must be authorized by the VA before services begin.

Depending on the Veteran’s needs and VA authorization, covered support may include:

  • Homemaker and home health aide services
  • Help with bathing, grooming, dressing, and personal care
  • Meal preparation and light household tasks
  • Support with daily routines that promote independence and safety
  • In-home respite care to give family caregivers time to rest and recharge
  • Other care services approved through the Veteran’s VA care team

Do you Qualify?

Veterans may be eligible for care through the VA Community Care Network when they are enrolled in VA health care and meet VA requirements for receiving care from a community provider. Eligibility depends on the Veteran’s individual health needs, location, VA appointment availability, and authorization from the VA.

A Veteran’s VA provider or care team will determine whether Community Care is appropriate and what services may be authorized.

  • The Veteran is enrolled in VA health care (or eligible to enroll).
  • VA primary care authorization
  • Referral from VA Case Manager or Social Worker
