What Is the GUIDE Program?
The GUIDE Program is a Medicare-covered initiative that provides families caring for loved ones with dementia access to dedicated, dementia-trained Registered Nurses — at no cost to you, ever. Through a partnership between PocketRN and Comfort Keepers of Tryon, qualifying families also receive 72 hours of paid respite home care each year.
Our dementia-trained nurses connect with you virtually via phone or video to monitor your loved one's health, coordinate care, and provide your family with the education and emotional support you need along the way.
Eligibilty Requirements
- Has dementia (a diagnosis can be provided)
- Is enrolled in Traditional Medicare Parts A & B
- Lives at home or in independent/assisted living
- Is NOT currently on hospice or a skilled nursing care resident
How do I qualify for paid home respite hours?
To qualify for the annual 72 hours of no-cost respite care, individuals must have moderate to severe dementia, as well as a family or unpaid caregiver — the caregiver does not need to live with the individual with dementia.
