In-Home Care Assessment: When, Why, How

An in-home care assessment is a critical early step in your relationship with Comfort Keepers. Even though you will have likely dedicated some time during your research stage to ask questions over the phone, review agency support resources, and correspond back and forth, the in-home care assessment is the foundation to creating a Plan of Care that is right for your situation, building trust, and developing a strong relationship.

So vital is this step that no service can begin until an assessment has been performed. Here are 5 reasons you, too, should embrace the role of an in-home care assessment:

  1. A good Caregiver match: By expanding on our questions to assess medical and behavioral conditions, care goals, likes and dislikes, previous careers, and even current pets, we get to know the whole person and assign a Caregiver who has the skills, experience, and personality for an ideal match.

  2. Get to know us: We appreciate the opportunity to get to know potential clients, family, and supporters, but it is just as important for you to get to know Comfort Keepers to ensure our values and model of care align with your needs.

  3. Uncover care needs: Often initial phone conversations focus on more immediate and obvious care needs but our asking more detailed questions, and allowing you to expand on your observations, can uncover less apparent care needs and factor in aspirational goals.
     
  4. Valuable use of time: Most in-home care assessments last about 30 minutes, but you can take as much time as needed to get your questions answered and provide information that can inform the Plan of Care.
     
  5. Aligned on care: As the foundation for your Plan of Care it is critical that all interested parties are aligned on exactly what services will be provided and how often. While care needs may change over time the in-home care assessment is a primary step to ensure everyone who wants or needs to be involved is able to weigh in and we are all on the same page, and expectations are clear before service begins.
