Person-Centered Plans
Each individual’s care is guided by a Person-Centered Plan that outlines active participation and specific goals tailored to unique abilities, preferences, and needs.

- Waiver Services: Chosen waiver services are detailed with information on waiver coverage, frequency, and duration.
- Plan Evolution: Reasonable accommodations and regular reassessment to ensure plans evolve with a person’s needs.
Accepted Waivers: We accept the Community Living Waiver, Person/Family Directed Supports Waiver, and Consolidated Waiver.
