Comfort Keepers & PocketRN - Expanding Support for Families Living with Dementia
At Comfort Keepers of Charlotte, we’re proud to partner with PocketRN to support families navigating the challenges of Alzheimer’s disease and related dementias. Through our partnership with PocketRN, the official CMS GUIDE participant, we support the delivery of Medicare-covered respite care under the GUIDE Model.
What is the GUIDE Program?
GUIDE (Guiding an Improved Dementia Experience) is a groundbreaking initiative from the CMS Innovation Center. It offers enhanced care services to individuals living with dementia and provides their caregivers with respite care at no cost to eligible beneficiaries, funded by Medicare. Under the GUIDE Model, PocketRN provides clients with peace of mind through regular nurse check-ins, family caregiver support and education, and 24/7 access to the care team.
Financial Assistance for Dementia Care
Eligible families may receive up to 72 hours of fully reimbursed in-home respite care each year, allowing caregivers to rest, recharge, or tend to other responsibilities—all while knowing their loved one is in compassionate, professional hands. Comfort Keepers works closely with PocketRN to coordinate these services at no cost to eligible beneficiaries.
Who Qualifies?
To be eligible for the GUIDE program through Comfort Keepers and PocketRN, individuals must:
- Have a diagnosis (or suspected diagnosis) of Alzheimer’s or other dementia
- Be enrolled in Original Medicare (Part A & B)
- Not be enrolled in Medicare Advantage, hospice, or long-term nursing home care
Caregivers must also be involved in annual assessments and complete a short questionnaire to maintain respite eligibility.

- Collect key information to confirm Medicare eligibility
- Schedule assessments with PocketRN’s clinical team
- Deliver respite care through our trusted, highly trained caregivers
