Community Care Program
Older adults in Illinois ages 60+ who receive Medicaid may be eligible to receive additional services through the Community Care Program (CCP). This program is designed to prevent unnecessary institutionalization of older adults who have difficulty with household and personal care tasks. Core services include in-home, adult day and emergency home response.
The Community Care Program provides services to any person who applies for the program and meets all current eligibility requirements. The Community Care Program is one of the 1915(c) waivers for home and community-based services under the Medicaid Program. Who is eligible?
People who...
- Are 60 years old or older;
- Are U.S. citizens or legal aliens;
- Are residents of Illinois;
- Have non-exempt assets of $17,500 or less (Your home, car, or personal furnishings are classified as exempt assets.); and
- Have an assessed need for long term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment).
What is the process of applying?
- Reach out to a Care Coordination Unit (CCU) to be assessed for the Community Care Program, along with other programs and benefits that may be available to you or your loved one.
Care Coordination Units are appointed by Illinois Department on Aging to provide local support to communities and help reach all that seek assistance.

- Once assessed by a Care Coordination Unit (CCU), and deemed eligible for in-home home care, you and your loved one have a choice of the home-care agencies that can provide a caregiver (also known as homemaker).

Does the Veterans Administration cover Home-Care Services?
If you need help with your daily activities, or you’re housebound, you may qualify for Aid and Attendance or Housebound allowances in addition to your pension benefits.
