A son in Iselin called me last month about his mother in Edison. She had been up four times the night before, and he was wiped out. The first thing I asked him was, "Why do you think you need 24-hour care?" He had assumed that was the only answer. It usually is not. The two services are different, the cost is very different, and the right call depends on what those night-wakings actually look like.

Here is the difference in plain terms. Live-in care is one caregiver who lives in your loved one's home for stretches at a time, sleeps there overnight, and gets required sleep and breaks built into the day. It fits a senior who sleeps reasonably well at night and needs help during the day. 24-hour care covers the clock with shifts, so someone is awake and on duty every minute. It fits when your loved one is up a lot at night, wanders, or has to be watched around the clock. Live-in is usually more than half the cost of 24-hour, because the overnight is paid as sleep hours. What live-in care looks like in an Edison home

A live-in caregiver moves into the home for weeks at a time. The same person, week after week. She has her own room and her own bed. She is with your loved one through the waking hours and sleeps in the home at night. New Jersey law requires that she gets an uninterrupted sleep period and breaks during the day. When she takes her vacation, usually about a week, we bring in another caregiver, and your regular caregiver comes back once she is off her break.

That consistency is the whole point. The same familiar face learns your mother's every routine, what she eats, how she likes her coffee, which chair is hers. 24-hour care covers the day with shifts. Because no one person can work around the clock, more than one caregiver is involved. We keep that number as small as we can, so your loved one still sees familiar faces and not a parade of strangers. Someone is awake and on duty in the home every minute of the day and night.

This is the right service when your loved one is up several times a night, needs help to the bathroom in the dark, tries to leave the house, or simply has to be watched at all times. The cost runs higher than live-in because the overnight is paid as full awake hours, not sleep hours.

How an Edison family decides between the two

The cleanest test is what last month actually looked like at night.

If your loved one slept through most nights and the real worries are daytime, falls in the kitchen, the stove left on, missed medications, sitting alone all day, then live-in is the right starting point. If she was up three or four times a night, or the family found her in the kitchen at 4 a.m., or she has tried to get out the door, then 24-hour care is the safer answer.
