A Medicaid Consumer Directed Program
Consumer-directed programs allow clients more choice and control over their own care. The clients may select, hire, train, supervise and dismiss their personal care worker. Clients decide whom to hire, what services are provided and the time of day their worker provides care.

1. What are the cares or services provided?
Attendant support – in home care and other supportive activities to assist eligible individuals to accomplish daily living tasks. Services include: Health Maintenance Activities - Those routine and repetitive activities of daily living which are necessary for health and normal bodily function. - An individual with a disability would carry out these activities if he/she were physically able. - Examples: bowel/bladder care, blood glucose monitoring, medication administration, vent care, G-tube feeding, vitals monitoring, transfers, etc. Anything you would teach a family member to do Personal Care– bathing, dressing, grooming, nail care Homemaker -routine light housecleaning, meal preparation, laundry, shopping
2. May attendants provide skilled care?
Yes. Every client’s care is overseen by a Comfort Keepers RN, who may train their single, dedicated caregiver to do the specific skilled job that is required (much the same idea as training a spouse to give insulin injections). Any backups, if needed, will be adequately licensed for the tasks they will need to perform.
3. Who directs the care and trains the attendants?
The client or authorized representative may choose, train, supervise and dismiss attendants. Comfort Keepers hires the attendants. Comfort Keepers provide training to the client and to attendants. The client may elect to do additional training or assume responsibility for most of the attendants’ training.
4. Who is the ideal candidate?
New or current Medicaid HCBS-EBD recipients interested in directing their own care.
5. What is the service delivery model?
Service delivery Services are provided by an attendant, under direction of the consumer, a parent or guardian of a child, or an authorized representative. The attendant must be employed by an IHSS agency (like Comfort Keepers). A consumer, parent or Authorized Representative can choose the certified IHSS agency. Comfort Keepers hires an attendant of the consumer and/or authorized representative’s choice OR can provide an attendant from our pool of current employees, with consumer and/or Authorized Representative approval
6. How does one become eligible?
The client must:
- Be eligible for the HCBS-EBD. Determined by the Dept. of Human Services Case manager,
- Be able to direct his or her own care or have an authorized representative to direct care.
- Submit a doctor’s statement certifying that the client or an authorized representative can direct care
- Complete an Authorized Representative Form, if applicable
7. Will the program helps qualified persons to receive services in an assisted living communities?
This Medicaid waiver is designed to help the elderly who have functional impairments that qualify them to receive nursing home care. Individuals elect to receive care services at home or in alternative care facilities, such as assisted living, instead of in a nursing home environment. In addition to choosing their care environment, under this programs individuals have the option to "consumer direct" or self-direct their own care services. Meaning, they are able to hire and manage some of their own care providers. As an example, they can elect from whom they receive personal care, but not necessarily who provides them with non-medical transportation.
Services covered under this waiver are selected to enable participants to maintain their independence outside of nursing homes. As such, personal emergency response services (PERS) and home modifications are covered, as well as a broad category of assistance referred to as In-Home Support Services (IHSS). IHSS includes help with home-related tasks, such as laundry, house cleaning, and personal care, such as bathing and eating.
This waiver's full, official name is Home and Community-Based Services waiver for Persons who are Elderly, Blind and Disabled (HCBS-EBD).
