A Unique Care Plan Created for Each Client
Comfort Keepers designs a senior care plan especially for each client. To do this, we start with a free in-home consultation. We meet with the prospective client and family representatives to gain a full understanding of their needs and become familiar with the client’s personality, preferences and interests. Working with the client and family, we develop a uniquely appropriate plan of care. The information we gain in this meeting also helps us match the client with the most compatible caregiver. Our experience has proven that a good client-caregiver match leads to the best outcome and quality of life for the client.
Family members may choose to be as involved in their loved one’s senior care as possible. Comfort Keepers works as a partner with family caregivers, keeping family informed about changes we observe in their loved one and recommending changes in the care plan to allow the client to maintain a high quality of life and independence at home.
On a regular basis, Comfort Keepers reviews the home care plan of each client to measure its effectiveness in supporting independence. We also review the performance of our caregivers, schedule monthly visits to clients’ homes, and gather feedback from clients and caregivers. Through these means, we strive to assure family members that their loved ones are receiving the best of care.
Alzheimer’s/Dementia Care
Comfort Keepers’ Memory Care program offers specialized in-home care for clients with Dementia or Alzheimer’s disease. Certified Nursing Assistants and Certified Home Health Aides, specially trained in Dementia and Alzheimer’s care, provide this service. The training prepares them with an in-depth understanding of how to communicate with, respond to, and care for persons with these conditions.
Memory Care clients are matched with caregivers who possess the training, skills and personality best suited to each client’s condition and circumstances. Memory Care can be scheduled for as little as an hour or two a week to full-time. We also offer overnight care to provide family members the opportunity to rest for their own well-being and that of their loved one.
In-Home Technology Solutions
The following technology solutions are available through Comfort Keepers to extend and enhance the in-home care and senior care that our caregivers provide:
Comfort Keepers’ SafetyChoice™ Personal Emergency Response System (PERS) gives family members peace of mind around the clock that their loved one can live safely at home. With the PERS system, a Comfort Keepers client wears an emergency call button on a pendant or bracelet. By simply pushing the button the client instantly summons help in the event of a fall or other emergency. PERS units are monitored 24 hours a day at the SafetyChoice monitoring center by specially trained personnel who dispatch help. Calls also can go out to 911 and family members. PERS brings help in the critical minutes, reducing the severity of injury or illness.
The Comfort Keepers computerized tracking system helps assure that clients receive the best quality of senior care and service. The system e-mails family members a care schedule, informing them which caregiver will be caring for their loved one on a given day. In addition, the system alerts the Comfort Keepers office when a caregiver is more than five minutes late for an appointment. A manager can then follow up on behalf of the client.
The MD.2 medication management system helps Comfort Keepers clients take their medications as prescribed, for optimal health benefits. The system signals a reminder of when it’s time to take each scheduled dose of medicine. A family caregiver sets the unit like an alarm clock and loads it with the medications. When the alarm sounds, the client presses a button, and the MD.2 dispenses a container of pills to be taken at that time. If the client fails to respond to an alarm, the MD.2 automatically alerts a family caregiver or other designated person. That person then follows through to make certain the medicine is taken.
